海倫王的問題是關於 "副詞的位置".

(A)副詞的定義:
副詞是用來形容動詞,形容詞,或是其他非名詞的詞類.副詞也可以用來形容一整個句子.

(B)副詞的位置:
副詞用在形容動詞,或是整句話的時候通常放在後面.
或是副詞落落長的時候,它也會被分開放(前面或是後面).
其他的情況就會放在前面.

但是有一個一定的原則就是:

在不破壞其他句子的結構下,要靠近被形容的字詞.

(C) 你的問題:
1. He is well past 40. (O - well要靠近past)
2. We couldn't very well refuse her. (O - very well要靠近refuse)

(D) 為神你是小妹?還在讀小學校嗎?

well ,adv, must be placed in a good, right or acceptable position in the sentence:-
(1)Her son is well past forty
=aged forty
Her son is past forty well
=Her son is past forty(and going very well)
=adverb;
Her son past forty(and considered very well)
=react/behave very well
=Her son behaved very well in aged 40 in right ,acceptable position
=adverb must be added in right position.
(2)We couldn't very well refuse her
=could we?
=could well help her --with good reason
=and we really know.
We couldn't refuse her very well---no object !?
=We couldn't refuse her (feeling) very well
=We couldn't refuse her (feeling) (in a) very well (manner)
=It may well be that she is sick
=We couldn't refuse her (who)(may very well ) be sick !
=She was very well received by us-----passive voice ..
=She was very well received/accepted by us-------passive voice with object
=No refusal but was very well accepted-------active voice
