According to Bronsted-Lowry theory on acids and bases, acid base reactions involve transfer of protons (hydrogen ions). An acid is a proton donor while a base is a proton acceptor.
acid base reactions involve H+ ions (bronsted/lowry and arrhrenius theories anyway). Since hydrogen atoms consist of a nucleus with 1 proton surrounded by an electron field containing 1 electron (in the neutral atom), to make an H+ ion, we remove the electron and all we're left with is a proton.
So, we could say (and we often do) that acid base reactions involve the transfer of protons. That is the exception to the "all chemical reactions are simply rearrangements of electrons and do not involve the nucleus" rule.

Semantics. An Acid-Base reaction does involve the transfer of H+, it can be argued that in this case it is acting as a hydrogen ion and not as a proton. So (c) or (d) could be correct answers.
What? There are no protons transferred from nucleus to nucleus in a chemical reaction. Sometimes we think of electrons being transferred in a redox reaction.
The exception to this is when we consider a hydrogen ion to be a "proton." Hydrogen ions are "transferred" in acid-base reactions.
HC2H3O2(aq) + OH- --> C2H3O2^- + HOH
In this case, a "proton" (aka hydrogen ion) is transferred from the acetic acid molecule to the hydroxide to make water. An acetate ion is left behind.
