EMR Electronic Medical Records, or EHR Electronic Health Records, are different types of systems for collecting and filing medical records for a patient. It was supposed to make the data stored on paper obsolete and allow doctors to have easier times transferring medical information about new patients to other doctors, and instead it backfired and created a system where your medical records with one doc who talks smack about a patient to be more easily maligned and basically doxed. Yeah, thought computers would make life better. Sadly, most chronically ill patients who don't mean big $$$ for the doctors are stigmatized, along with females (hysterical psych cases), POC (poor/likely drug seeking), disabled (malingerers/fakers), and so one. Docs have been selling out patients for decades now, and the Electronic records make sure that your dentist is privy to your diagnosis of ADHD, as if that mattered at all when you just need to get your teeth cleaned. I Know TMI, but I've worked in it, and it's just ironic that what was supposed to make healthcare faster and more streamlined is corrupted by the people who are entrusted to care for your health, not run a gossip/rumor mill with the electronic data. I've seen RN's type in that patients were drunks after THEY messed up and gave contraindicated meds to a patient they didn't bother to read the chart on. It's scary.
