A spouse would generally not be required to testify against their spouse, but that same protection doesn't extend to other family members.

If subpoenaed, they could be made to testify. But they aren't expected or allowed to "make a statement as they aren't a party to the action.

No, there is no mandatory requirement for either defendants or victims families to make statements at a trial.
