Is all drugs approved by FDA safe?

In history, was there any drug previously approved by FDA but later found out to be
risky and wasn't then prohibited?

OK Nobody seems to be answering my question. I'll try to raise the question in a different way. What is the factor that usually makes one drug approved while the other is disapproved by FDA? When a drug is approved by FDA, does it imply that it is legally possible, but very unlikely in terms of probability to cause a side effect that will outweigh the benefits or a side effect that does not worth the benefits it gives?

There have been many drugs that were approved but then found to be unsafe.
Thalidomide comes to mind.

It can take years for some problems to manifest.

Death can be an acceptable side effect of drugs. So I guess it all depends on the context in which you are using the drugs.

No. In fact most are not necessarily safe. Have you read the possible side effects of drugs?
