It’s hard to put one’s finger on what’s the matter with him. Obviously he was never really “all there,” but age, glands, injury, illness, stress, diet, & substance abuse could all be factors. He’s definitely become worse.

You might be senile yourself. The term "senile" is no longer used. Instead, we say "he has dementia." The medical term is "senile dementia," and for some reason, "senile" is regarded as pejorative when it just means "old," and "dementia," of all things, is politically correct.
