It means you should honor your father and mother [which includes caring for them in their old age], and help your siblings and other relatives to the best of your ability.

It means...of all the things that you do which brings you rewards , your responsibility towards your family should be done first.

We, Chinese, have an ancient proverb, which says 不孝有三，無後為大
