It can't be done.

1. When a person apply for a visa for medical purposes, the person must submit the admission or acceptance letter from the hospital inviting the treatment.

2. The person must have adequate financial resources to support the trip (which means the medical cost as well). Given as a CSSA recipient, it is impossible to provide.
