I'm not just going to give you the answer, then you won't learn anything. I'll show you how to do it for yourself.

You are given the general form for vertex form :

f(x) = a(x - h)² + k

This is called vertex form because the vertex is at the point (h, k). Which is given to you at point (4, 9).

So now we have:

f(x) = a(x - 4)² + 9

Next we need to solve for "a". We have one other point (5, 14), which can be substituted in for your "x" and "f(x)". This leaves one variable left to be solved for:

14 = a(5 - 4)² + 9

Now simplify and solve for a:

14 = a(1)² + 9
14 = a(1) + 9
14 = a + 9
a = 5

So your equation is:

f(x) = 5(x - 4)² + 9
