Therapist confidentiality ?
Are therapist s notes accessible to subsequent therapists of the patient to read ? Can a patient request certain information be "off the record " and not be written down during the therapeutic process ?

Hippa laws require the patient to give permission to share records. But does not give you the right to edit which records are offered. In other words its all or nothing.
i'm told that the general practice is that the notes a therapist takes do not leave that therapist unless the client specifically requests.
and while you as client can ask for anything you want in this area -- therapist has the right to decline to see you at all if your requests would seem to make it difficult for her/him to properly serve you.
