usually the "get" part means to succeed in causing you harm. They have targeted you for punishment. "being out to get" tends to mean actively trying to cause you trouble, pain, or other bad things.

means the person is actively trying to hurt you physically, socially, economically, etc....out to harm you or best you. they are the bad guy
