Is it "dein Name" or "ihr Name"?

We are making a sign up sheet for the german club and they wrote on the sheet ihr Name (Your Name) but wouldnt it be dein Name because we need to have more context in a sentence in order to use ihr?

It depends how formal or polite you want to be. "dein" is informal (du form), used for friends, family, children and dogs. "ihr" is formal (sie form) and is used for people you don't know or those in authority.
