Does the novel have to be popular to become a graphic novel; or can the author ask for it to become a graphic novel?

Yes, the author asks for this. Alternatively, an artist might express interest in such a thing. It doesn't mean anyone will be interested in publishing one, given the profit margin of such a thing.
