How much does apple charge to replace a water damaged iPhone out of warranty. If they replace it at all?

Depends on what needs to be replaced in the phone. Probably, they'll say that the mainboard and screen has shorted and you'll have to replace the whole thing which costs almost same as getting a new one. I would not take it to Apple but go to a phone repairer and have it repaired with OEM parts at reasonable price.

If it's out of warranty it would cost the cost of a whole one.
You can get used 4S phones for like $50 on eBay.
