Since July I've been passing out multiple times a day, always dizzy, pounding headaches,blurred vision,&memory problems. What could this be?

I know I've been having symptoms for almost a year now. My pcp kept refusing to give me a referral. She made me try different things like lowering/ stopping some
of my meds,& getting new prescription for glasses. I'm on disability for anorexia, I have Medicaid so for any specialist I need a referral. I finally got it but it was after me yelling at her since October when I started seeing her and after I did all that she still didn't want to give it to me she wanted me to go to the balance center it put me over the edge because she's messing with my health. So I got my referral for neurologist. I see the neurologist this week on Thursday. I'm just scared he's gonna treat me horrible like every other doctor does because of this town &I'm judged for my illnesses. But I just wanted to know if anyone had any idea what these symptoms could be. I'm always dizzy, I can't walk straight, I pass out everyday, pounding headaches for hours on a daily basis, hiccups everyday for at least an hour, and it takes me for ever to type because I can't spell and symbol and emojis end up in there somehow(I'm a good txt &typer), blurred vision, memory problems, and I walk into things and fall into things. I'm Scared that with my pcp waiting so long to give me referral that it might be serious and worse. I have gotten good at knowing when I'm Passing out and stay close to my bed or recliner so I can get to those before I hit the ground and hit my head for the millionth time. Any one have any ideas?
