Use the formula to find the vertex, and work backwards from there:
y = a*(x-p)^2 + q, where (p,q) is the vertex, and a is any constant that is used to change the size, but not the shape, of the graph.
Subbing p = 0 and q = 4,
y = a*(x-0)^2 + 4
y = a*(x^2) + 4
You need one other point on the parabola to determine a.
