Should I ask my gf out on a date or wait and see if she initiates it?

I know this may sound a little childish however when I ask my girlfriend out sometimes she may get annoyed cause I ask her all the time and I don't want to sound selfish by asking her. She just had a death in the family and I feel like I would annoy her if I asked. Should I ask her, or wait to see if she initiates it? I was thinking of saying " I understand you just had a death in your family and you want to be with them however I would really appreciate it if you seen me cause I worry about you , if you would like to go out?".
