I know you probably hate me and want nothing to do with me. I'm not asking you to speak or even to consider me as a friend but I do need to know that you forgive me so that I can move on and be ok with myself ( feel free to add additional things)

It seems a selfish message to someone that you have hurt in some way.
Rather than thinking of your own ratification on the matter, think of first, why this person should forgive you from their perspective and why you would forgive someone that had hurt you in the same way you seem to have hurt the person the message is for.
Forgiveness from another will not help you move on, until you can forgive yourself and make or at least aim at making amends. We all make mistakes in life.
