How should I approach my manager for a reference?

I'm trying to get a reference cleared off one of my managers i use to work with, it's proved to be a difficult task to get a hold of one of the other managers by phone but I have the other one via Facebook. How should I approach him and what should I say?

Approach them in person (if possible) and ask them for a reference letter or ask them to be a reference. Or email them and ask them that.

"Hi. Mr. Thumbgrope! I'm applying for jobs and am writing to ask if you mind my listing you as a reference."

Simply state you are seeking employment and would appreciate her/him giving you a reference. The worst that can happen is either no reply or "no."
