How can I change my spouse visa name?

I have married with my Malaysia citizen husband for almost 3 years. We both are Chinese and married in Malaysia .For some Chinese superstition reason, i am planning to change my name, also on my Hong Kong ID card and passport.
After changed name, how can i change my marriage certificate information? Is that JPN.KC08 form support this kind of change? And what others document should i provide to?

I have a five years spouse visa which will be invalid in June 2018. After i got my new passport with my new name, expect the Deed Poll , what else documents i should provide to change my spouse visa name?

First, to change your name, you need a court order. Use the court order to change the name on your HK IC, passport, driver's license, bank & other financial accounts. You cannot change your marriage certificate - you use that plus your court order to change your name on later documents, such as passport & applying for your new/renewed visa.

Application Conditions
1. This application is only to correct errors made at the time of marriage registration.
2. Applications may be submitted at any NRD counter.

Required Documents
1. Form JPN.KC08 that is completed.
2. MyKad (photocopy of MyKad if application is by post).
3. ORIGINAL passport (photocopy of passport pages with personal details if application is by post).
4. ORIGINAL Marriage Certificate.
5. Statutory declaration relating to error to be corrected.
6. Other supporting documents (if applicable).
