Afraid of the future. What can I do?

I have a goal, that I really, really, really want to accomplish, as much as I want to breath.
But sometimes, I think it isn't possible, and I want to cry. I want take a phd in the us, and am scared of not receiving the visa. And then what? I'll be stock. I no im smart enough for it, but what if everything is against me?
