People normally limit how much of their muscle strength they use to avoid damaging their muscles or joints.
Under certain circumstances, people can supposedly bypass that limit and use far more strength, to the point they can tear muscles or break their own bones with the force they apply.
You occasionally see stories of people who experience this effect under extreme conditions; it does not seem too far-fetched that someone with mental problems who's in an unusual mental state could do something similar.
It can also happen with some diseases, eg. rabies victims lose sensation and can develop extreme violent behaviour and super strength, to the point of damaging their own bodies.
