Inspection of tax free items at Japanese airports?
Do I need to hand carry items for inspection by Japanese customs or do they inspect it before I check in luggage? It would be inconvenient if customs require me to show them items I already placed in my checked in luggage. Would I be asked to pay back the tax refund if the Japanese customs do get to see my tax free items?

Huh? There isn't any export duty. You'd check in your luggage for international flights, and your items will be inspected in the country you land (disembark) in.
If you are coming into Japan, everyone picks up all their luggage, and goes through immigration and customs. There are carts. Then, if you have a domestic connection at the same airport, you take all your luggage to . . . well, actually, I think you have to take it all to the check-in counter, now that I think of it. There's no place in Narita to re-check your luggage right after customs.
I usually have to switch airports for domestic connections (not always; but when I came back through Haneda, we had to stay overnight, so we took all our luggage to the hotel). If you are coming in through Narita, you pick up all your luggage and take it with you to Haneda and re-check your luggage at the domestic counters for a domestic flight.
If you are just in transit, I don't think anyone checks your luggage. You don't pick it up, anyway.
why would you have to pay the tax back??? you got them tax free because your a tourist who will be leaving the country.
You're carry on is inspected when you pass through the security gates, that's after you check your luggage.
