thank you note help?

I'm graduating high school next week and got a gift for my favorite teacher but I'm wondering what should i write in a thank you note for them? i had this teacher for all of sophomore year, half of junior year, and all of this year (student teacher for him). we've developed a strong relationship over the past few years, he wrote my letter of recommendation, and overall he's had a huge impact on my life.
