Fill Out the UNIFORM SALES & USE TAX CERTIFICATE—MULTIJURISDICTION?

That form is for companies that sell products in more than one state. If you plan on selling products, you're required to charge sales tax. The benefit of this certificate is that it allows you to simply file this single one to charge sales tax in the various states you'll be selling in, without having to apply in every single state.

If you aren't selling products, then you won't be charging sales tax and this form is unnecessary.

It is for businesses that sell a product so that they can buy that product exempt from sales tax.

If you have a business and you plan on reselling the products and collecting sales tax form your customers, you can fill it out.

If not, filling it out and trying to use it, would be considered fraud.
