Need help for Oyster card application. 16?

I started filling in my application for a new oyster but i need to download a verification form but i can't find where to download it. I need to finish it soon or it will expire.

You download it from the application form - but it won't be there unless in filling in the form you put something which means that you need it.

It will be obvious during the application process IF you need to download one.
