這句是否會產生歧義？

沒有歧義 - 可能你對subject主體的定義或樣子仍不太理解 - 不是凡是名詞或人名便是subject的。

the man between my parents中的subject主體(即句子的主角)是the man，my parents在句子(或片語)中是附加解釋跟這個the man的關係是在你父母兩人中間(between)

接下子句/片語holding an umbrella仍是必定附在the man這個subject上的形容或附加解釋。

接下的子句/原句is x這個is才是全句的動詞，而主角仍是the man...is my uncle.

因此不會誤會的。

反而句式上
In this photo後應要加上"，"因為這片語亦是附加描述the man的其中一樣。

因此其實全句只主意思是想表達
The man.....is my uncle Jack.

The man (那一個男人): (who is) in the photo,
The man (在相片中的那一個): (who is) beween my parents,
The man (有甚麼特徵): (who is) holding an umbrella,

沒有歧義，因為你有parent，有uncle，冇得錯，除非你只有某人一詞，誰人担遮冇關係。

嚴格來說，這句說話的文法是錯的。

逗號內的一般只係逗號前的名詞的另一個寫法，以作解釋。
例如：
(1) This is my father, Edward Chen.
(2) My father, Edward Chen, is a very nice man.

用來加以描述句中主語 (subject) 時，我們一般應用關係代名詞 (relative pronoun)。但用時要小心，以勉產生歧義。

你的句子可寫成：
In this photo, the man between my parents who is holding an umbrella is my Uncle Jake.

不過這樣寫是有可能產生歧義的。由於雨傘只有一把，而男人站在父母中間，一般人都不會誤會。但想像若 Uncle Jake 只站在父親一人旁邊：
In this photo, the man standing next to my father who is holding an unbrella is my Uncle Jake.
這句明顯產生歧義，因為不知拿傘子的是父親還是 uncle。

所以你的句子最好寫成：
In this photo, the man holding an umbrella who is standing between my parents is my Uncle Jake.
