加拿大學生簽證問題

10月既時候申請study permit,
今日收到一封信

寫住: Please give us more detail on the medical examination ie; IMM1017 form, Name of Panel Physician and email address, where and when the medical report was sent to - Beijing or Manila.

我搵到IMM1017, 係做完body check時比既 證明我做左 診所有講過話可能領事館會要
但上面冇email address
而且我唔明where and when the medical report was sent to - Beijing or Manila.
份report唔係應該send左上香港加拿大領事館? 點解寫住beijing or manila?
再講份IMM1017都冇講過呢樣野

其實係咪只需要比IMM1017? 希望有人知道, thank you.
