English Grammar - Were, Did, Has, Have?

I have been confused by the following sentences.
It would be a big help if someone could explain the difference. Thanks.

Q1:
She is going to have a baby.
She is going to has a baby.

Q2:
Were you in London yesterday?
Did you in London yesterday?

Q3:
Are you in London now?
Do you in London now?

Q4:
Does Phil get a headache?
Has Phil got a headache?

Q5:
Did Ann have a lesson yesterday?
Has Ann had a lesson yesterday?

The difference between the correct sentences and the impossible ones does not depend on the respective meanings of be, do, or have, but on the fact that you failed to study the verb forms which are related to this exercise - a very simplistic one if you open your book, with the exception of Q5 which requires some understanding the difference between simple past and present perfect.
