功課問題,聽日交....HELP~~

In my determination, these questions are asking 渡輪相撞後可產生的問題, but not 什麼問題導致渡輪相撞.
What problems might be caused by the ferry collision VS what might be the problems leading to the ferry collision.

I guess the structure of the questions are not quite correct.

1) What problems might cause the ferry collision?
a. Careless of the captain.
b. Poor weather condition - foggy or dark
c. Someone was not following the sailing rules

2) How can we avoid the problems caused by ferry collision?
a. Reinforce passengers to wear life jackets at all time while on a ferry
b. Consider if more safety equipments should be installed to the ferry, like seat belts.
c. Keep objects and bags in cabinet just like in airplane so things won't fly around and hit people in case of crash.
d. Chairs should be secure to the floor with the same reason.

3) Is there any way to help solve the problem caused by ferry collision?
a. Re-educate all the sailors and captains. Addition standardize test should introduce.
b. Upgrades the sailing instrument like radar. Signals to notify sailors or captions when the radar detects an approaching object.
c. Police reinforcement should be strengthened in the sea as well. With more patrols, people tend to pay more attentions to rules. Increase the rate of violation penalties might help too.

3)Add more saving clothes.

At approximately 20:20, the Sea Smooth crashed into the Lamma IV on the port side.[4] The bow of the Sea Smooth hit the stern of the Lamma IV, rupturing the ship's two watertight compartments and thereby allowing them to quickly fill with water.[5][6] The ship soon capsized with the rear sinking into the sea. The vessel reportedly went down so quickly that more than 100 passengers were thrown into the water unprotected despite the abundance of life-vests on board.
