I suggest you delay your interview.
In answering your questions first:
1. Yes. The information should have included with your interview information.
If not, here it is:

2. If you can, bring everything, such as every single document or form you have sent to the National Visa Center (even it is a copy), proof of relationship, I-864 (even it has been submitted), passport, photo.
3. It will be primarily focus on your relationship with the sponsoring relative (brother/sister) and the information you have provided.
4. If the interview has no outstanding issue, you will be asked to submit the document. However, there is no way to tell how much time it will be delayed for visa process.
5. Your criminal record is sent directly to the U.S. Consulate by the Hong Kong Police.
Why I ask you to re-schedule is you don't have enough time to complete everything for interview. With less than 5 business days, it seems impossible for your medical examination to be done.
Beside - it sounds like you know nothing about the entire process (and apparently this happens to your relative as well). So by delaying your interview schedule will allow you to have adequate time to prepare.
Go to the website and contact the Consulate for re-scheduling:

1. You will need a new criminal record. However, due to the way how the Hong Kong Police works, the U.S. Consulate (or the NVC) must provide necessary document to your family in case this happens.
So unless you are told, do nothing.

As soon as you are not married and you are under 21 years old, your eligibility will not be impacted. However, you should have provided your child information to the Consulate already.
