Dear Mr. Tam

Our Maintenance Department Staff ( Mr. Lee ) was injured yesterday ( around 19:00 ).

M. Lee was injured by the tool when preformed the job duty. He went to hospital for treatment immediately. He reported me that need take a leave for 3 days.
The tool has been checked by me on today's date. We found out that the tool has damaged therefore ken has injured. ( Please see the attached photo for your reference. )
I will checked all maintenance staff tools as soon as possible for prevent the case occur again. And, I will reminded our team that need to report the tool status as soon as possible if find out any problems.
( We found that the tool no any damaged that was Mr Lee careless to use the tool. I will remind him that need to care to work in the future. )
The hospital injured report will be attached to Human Resource Department when i get the report.

Dear Mr. Tam,
This is to inform you that Mr. Lee, the staff of Maintenance Department was injured at 19:00 hours yesterday. Having doing his job, he was injured by the tool accidentally and was sent to the hospital for treatment immediately. After discharged, he reported to me that he needed 3 days sickleave. Owing to this, I had checked the tool on question today and discovered that the tool wasn't functioned properly as the result Mr. Lee being injured. Please see the attached photo for your reference.
In order to prevent from such accident happened again, I'll check all the tools as soon as possible and remind all staff of Maintenance Department to report promptly to me if there is any problem regarding the tools.
In addition to that, if I found out the fact of the accident were due to Mr. Lee's careless in using the tool and hence he would be reminded to use the tool with extra care.
Also the hospital's discharged report will be sent to Human Resource Department as soon as being received.
