I can't get rid of my nervous habits?

Now, I'm not a person without confidence, but I just can't stop these nervous habits. It started a few years ago when I would twirl and twist a few strand of my hair. It would continue throughout the day. After 6 months or so, the certain area of hair I often played with started to get lessen. Scared I would be going bald, I forcefully stopped myself from twisting my hair. Now, few years later, I'm starting to twist my hair again, and this time I also got the habit of touching the joints of my ring fingers and my pinkies. I've touched my joints so often that they are starting to swell dramatically and it hurts when touched, but I still can't stop. Maybe you can give me some advice?
