confirm有確定，被磴實之意，to be confirmed即待一件事被肯定之義，這和待定有點差別，待定有等待一個決定，個人應為更好的翻譯應為to be determined

'to be confirmed' is correct
'to be confirm' is grammatically wrong.
If you need an adjective, you may consider 'tentative' if you mean 暫定, e.g. It is a tentative date/ venue, etc.
You can say the date is to be confirmed.

TO BE CONFIRMED有待証實/確認/確定

這句本身是某件事或決定以被動語態表達告訴讀者
所以to be+past participle.
這是必定這樣寫才合文法，你的另一個寫法基本上是錯的，所以不是那一比較好的問題。如果你採用主動語態亦可以，不過可要加上subject.

只可用 TO BE CONFIRMED, 因為 TO BE CONFIRM 是錯的
