How much is too much when working knots out of someone's back and how to do it properly?

My boyfriend has so, so many knots in his shoulders and back. I just recently noticed it, and I would like it if I could work them out for him. Apparently they don't hurt him on a day-to-day basis, but are incredibly painful when I try to massage them and poke them just right.
I think some of them have been there for a very long time, and a lot might be caused by his guitar playing and/or gaming.

How much is too much when I work on them?
When they are gone/relaxed, it shouldnt hurt when I press the spot right?
Are there any precautions I should take so as not to actually injure him? I've heard you should drink a lot of water after having a knot worked out...?
What would you guys say is the longest/ideal amount of time I should spend on them at one time? I don't know if he would tell me when it became too much, and I have a tendency to work at a knot until its gone, then move on to the next one and just keep working at it....

Remember that everyone has a different level of pain tolerance. Now my husband has a high tolerance to pain. But if I try to massage his back like "I" like to have mine done he get's all annoyed at me cause he says it hurts too much.

I think to begin with, shoot for a total of 30 minutes. If you do this every night you could then start working up in time until you have gotten all the muscles to relax.
