As you have not specified which University and which programme you are studying now, I cannot offer point-to-point advice. However, I can share some experience and comments for you.
The best strategy is to contact their Dept. office ( the best of the best is their counsellor ) about the change of university and their individual requirements for shifting. They have internal guidelines and would share with you. Either phone call or direct visit are possible at the early stage, but if you really decide to change, please submit by yourself and have formal dressing.
You are suggested to visit their websites ( in particular two desired Depts ) and have some basic ideas on their course structure, professor names, etc, before application.
The experience I can share is:
I know a boy who was a Science student in HKUST. He failed a core course twice and he was forced to withdraw ( better say he was kicked by HKUST ). However, the next year he became the Science student of CUHK! Is it crazy?
I am sure he used his AL results to sell himself or use learning atmosphere as a point to account why he studied so poorly in HKUST. Once CUHK trusted what he said, he could join.
This story tells me even there is no official guideline to the public, we need not worry whether the req. is very high. From the above case, it can be much lower than expected!
