Form of address for a woman who is a Professor and a Dame?

When addressing a woman whose title is Professor Dame Jane Doe, should we say -

"Dear Professor Dame Doe"
"Dear Professor Doe" or
"Dear Dame Doe"

Thanks to Bohogirl and William for the Debretts website. In fact I checked it before posting this question. The Debretts website does not advise how to deal with a woman who is a Professor and a Dame at the same time.
