This is my fault. 強調的是造成錯誤的責任.

所以你要選擇你要表達的正確詞彙. 係我錯了 - 應該是說責任吧

文法上;兩句都對, 口語上第一句比較常用.

既然是你的問題, 當然要加個 me 啦

兩種不同的文法處理。
我會用第一個，因為你是請求他人幫助「你」解決你的問題。第二句是半命令式，例如一位上司有禮貌地叫一個下屬解決一個公司的問題，便會這樣「不及物」地說。但你是有錯在先，你的中文講法要修飾一下才出口吧。

有些動詞後不可加to的,文法上稱infinitive without to如see, make,need, dare等, help 就是其中一個, 所以不可用to字在它後面。
