I assume you are applying for a non-immigrant visa (as I-134 does not apply to immigrant visa).
I-134 is not filled by you, but the person who sponsor you. So what you have to do is ask that person to fill it.
However, if I can, unless it is specified in your application that you need one or you apply any dual-intent visa (such as H, K), don't use this
form as you can because:
1. You disclosed your tiered relationship in the United States, which is a sign of intention to stay and result an increased chance of visa denial
2. The sponsor's failure to answer the question truthfully can result in permanent ban against you for any U.S. visas and your sponsor may subject to penalty and imprisonment.
