Re : Explanation of My Absences
I am sorry for the rate of my attendance is less than 80% in this academic year. This was due to a serious of my illnesses and my doctor asked me to stay at home more.
I herewith send you the letter of my medical certificate for your perusal.
Finally, I should be very grateful if you would kindly consider the reason of my absences.

應該是:

for the rate of my attendance which is less than 80%--------正 確

出席率未達標是不能領取証書，而且即使有任何合理的理由亦不能以此為申請理由，所以你的情況即使寫求情信亦是多餘的。
