i am going to write a similar sentence in my personal statement.

Familiar with C++ and JAVA proved by my final year projects.

Familiar with C++ and JAVA proven by my final year projects.

Familiar with C++ and JAVA, proved by my final year projects.

Familiar with C++ and JAVA which is proved by my final year projects.

Familiar with C++ and JAVA which are proved by my final year projects.




If none of them is correct...could you please suggest an appropriateone?

Which is the most correct?
Your written sentences are not quite correct because their first part 'Familiar with C++ and JAVA' is not a noun. It has to be in the form of a noun so that it 'is proved by my final year projects.'
My suggestion :
(1)
My familiarity with C++ and JAVA as proved in my final year projects.
This sentence keeps your original ideas and express them in an appropriate way.
If you are not just familiar with C++ and JAVA, but are proficient, skillful etc in them, use the nouns proficiency, skills accordingly.
(2)
You can also write like this without using the noun form of familiar :
I am familiar with C++ and JAVA as proved in my final year projects.

I presume you were writing a resume, for yourself or someone else...

So...
Proficient in C++ and JAVA.
or...
Recent projects done in C++ and JAVA.
or...
Completed several projects last year in C++ and JAVA.
