credit slip and payment slip

I live in UK, and I open a bank account there. Today, I have just received a cheque book, but I don't know what is credit slip and payment slip? What are they? They are enclosed with the cheque book.

Credit Slip - fill in when you want to put money (e.g. deposit cash) into your bank account
Payment Slip - fill in when you want to draw money (e.g. draw cash) from your bank account
