首先要說，who後面並不一定要用bare infinitive
如：
Who are you?
並且who後面並不一定要用單數形，或複數形動詞
如：Cambridge Dictionary就有以下例子
Who's she?
Who are all those people?
"It looks as if Scotland are going to win." "Who cares?".

而且，
yahoo字典就有一例：
Who has borrowed my pen?
誰借了我的鋼筆?

注意：是用has（單數形動詞）

所以，看來Who really needs our help?和who really need our help?

其分別是 ：用needs是問者已非常清楚，答案只是”一個人”
