親手遞辭職信，要寫自己地址嗎？

如果親手遞辭職信，還需要寫上自己的地址嗎？
以下的辭職信可以嗎？

Dear Mr Chan,

I regret to inform you that I decided to resign from my present position as Administration Assistant with effective from 4th August 2009 of 4th September 2009. (如果比1個月notice, 請問日期這樣寫正確嗎?)

Thank you for giving me a chance to learn and to gain the valuable experience in ABC Company. (需要寫埋部門名嗎?) I hope my resignation would not cause you much inconvenience.

Thank you for your kind attention and would appreciate if you could let me have a reference letter before I leave.
