Obstructive sleep apnea (OSA) is a disorder characterized by collapse of the pharyngeal airway during sleep. A sleep apnea event is defined as a 10-second cessation of airflow. The presence of continued ventilatory effort characterizes it as an obstructive apnea rather than a central apnea, in which airflow and ventilatory effort are both absent. Sleep hypopnea, a related condition, is a reduction in airflow by a defined amount, accompanied by a desaturation of 3%to 4%. Sleep-disordered breathing (SDB) is a more general term used to encompass both sleep apneas and hypopneas. Sleep apnea-hypopnea syndrome (SAHS) is used to define patients who have a sleep-study-based diagnosis of sleep apneas and hypopneas associated with clinical symptoms of the disorder. Examples of sleep apneas and hypopneas are shown in Figure 71.1.
SAHS is a common disorder. A large epidemiologic study of the prevalence of SDB in middle-aged Americans found that 4% of men and 2% of women have SAHS, defined as a positive sleep study with appropriate clinical symptoms. Abnormal sleep-study results without the presence of symptoms were found in 24% of men and 9% of women. Other epidemiologic studies have found similar degrees of prevalence.
Several conditions predispose an individual to sleep apnea. These include age, male gender, obesity, increased neck dircumference, use of alcohol or sedative or hypnotic medications,
and craniofacial abnormalities, such as retrognathia. A recent study demonstrated that certain craniofacial abnormalities, such as retrognathia and a decreased intermolar distance (resulting in a high, arched palate), when combined with the body mass index (kg/m2), were strongly predictive of SDB.
