http://en.wikipedia.org/wiki/History_of_the_periodic_table), the first person to list out elements in a table is Lavoisier. However, the idea of a "periodic table" does not exist till Alexandre-Emile Béguyer de Chancourtois started noticing the periodic nature of elements.
In 1869, Dmitri Mendeleev had a formal presentation of a table within a paper entitled "The Dependence Between the Properties of the Atomic Weights of the Elements". Hence, this is the first periodic table.
This table had been reprinted and somewhat rearranged among books and documents. The followings are images of this table from various sources in the web:
