Exhaust from vehicles is a hot gas (mostly carbon dioxide and nitrogen). The density of which is lower than the sorrounding air because of its elevated temperature.
As such, light is refracted when it passes through the patch of hot exhaust. But the exhaust is in continuous motion, thus the image seen is flickering.
