"How do you do? <------It is very formal.( When you first meet someone, you say this. It usually requires no reply.
In some grammar books, they say you may reply with" How do you do?".

Note: In modern English, that refers to " How are you?".

1) I am doing okay (oral).

Note: Just tell that person what you ARE DOING. It all depends.
