As 'since' means the event (being a good student) happens after a past event (going to secondary school),'go' should be in past tense and 'be' should be in present perfect tense.
'year'(singular) and not 'years' for the adjective 'every' should be used.
Answer:
I have been (present perfect tense) a good student every year(singular) since I went(past tense) to secondary school.

2)i ___(BE) a class monitor every _______(YEAR/YEARS)
Similar to 1), 'year' should be used after 'every'.
We can use two different tenses for 'be', giving different time that the event (being a class monitor) takes place.
Answers:
a.I have been (present perfect tense) a class monitor every year.-(I was a ...in the past, then again the next year, and again, and is still a.. this year.)
b. I was (past tense) a class monitor every year. (I was a ... for a few years in the past.)
