Hi, firstly ... u need to input all informations at http://202.64.38.188/niv_ds156c.html in ur pc After that, u click {continue}, it will show the completed form with your informations; then u can print-out it for the interview or future use.

同埋我無打印機.....有咩方法可以儲存157同158d資料""我填哂張表d野""跟住儲存就話唔可以填野再儲存....咁有咩方法-00-..""唔該!!!

係填左先印.
要填份form, 你部電腦一定要有:
1. IE at lease 5.0 + Service Pack 2 (if you are using Windows)
2. Adobe Acrobat Reader Version 5.0 或再新的version
好緊要一件事...你份form一定唔可以褶過..
所以print完之後, 你最好用個file or公文袋入好佢!
